Staff at the Lincoln Institute for Health have been awarded a funded doctoral studentship by  NIHR CLAHRC East Midlands. Working in partnership between the Universities of Lincoln and Nottingham, the doctorate will explore  the effectiveness (and cost-effectiveness) of different models of Community or Care Navigators supporting older people with a number of long-term conditions (comorbidity or multimorbidity). Such a role is likely to be essential over the next five years if appropriate and quality care is to be delivered to older people in a health and care environment that is becoming increasingly fragmented.

 

The Care Navigator  role has been delivered in different ways, but their core role is to  identify available services, signpost and support access to particular interventions and facilitate service integration through their role as link worker. The successful student will work alongside Lincolnshire West Clinical Commissioning Group, Lincolnshire Adult Social Care and a variety of third sector organisations; in particular, Age UK Lincoln.
